Registration Confirmation

When you submit your registration form, we then process your credit card payment. If the credit card charge is successful, we then send you an e-mail confirmation. If the credit transaction is unsuccessful, we will promptly notify you by phone, fax or e-mail.

Payment Information

Payment in U. S. dollars may be made by personal or company check, money order, or credit card. Mailed checks must be payable to the American Society of Law, Medicine & Ethics. Please print out and submit the PDF Registration Form along with the check. Online registrations must be paid by credit card. Government vouchers, wire transfers, telephone or electronic mail registrations will not be accepted. International payment must be by credit card or International Money Order.

Cancellation Policy

Registration cancellations must be received in writing on or before May 25, 2006 & are subject to a $50 processing fee. Refunds for this program will not be permitted after June 9, 2006. If you would like to send a substitute, please call Katie Johnson at ASLME to arrange.

Special Needs

Special Needs If you have special needs addressed by the Americans with Disabilities Act, notify ASLME at least 3 weeks prior to the program.
