It can be used by the New Hampshire Department of Health and Human Services to compel individuals suspected of having a contagious disease to undergo medical treatment to render them non-contagious.

This Form was prepared by the New Hampshire Office of the Attorney General in 2003 as part of the state's emergency preparedness activities. It can be used by the New Hampshire Department of Health and Human Services to order an individual suspected of being infected with a contagious disease to undergo a medical examination, including production of such specimens as determined necessary by medical personnel to determine the presence of a communicable disease.
